- Most probable diagnosis is benign bilateral dermoid cysts (differential diagnosis: haemorrhagic corpus luteum, endometrioma and mucinous cystadenoma).
- Complex ovarian masses that contain both cystic and solid components are mostly dermoid cysts. The cysts themselves are usually solitary, spherical or ovoid, and echogenic. They contain a solid area of hair and calcified material along with 'buttery' sebum which tends to layer anteriorly due to its lower specific gravity. Sonographic diagnosis of dermoid cysts has 84% sensitivity and 90% specificity.
- Infertility is caused by disturbance of ovulation.
- The best method for removing dermoid cysts is by laparoscopic ovarian cystectomy. Preservation of the ovary should be the aim of surgery and a thorough lavage of the peritoneal cavity is mandatory.
- The prognosis for subsequent fertility is very good.
