Your fictional patient, a 35 year old woman, consults you because of her inability to become pregnant, a 'problem' exacerbated by her complaint of painful intercourse and frequent tiredness. She also experiences pain during defaecation, as well as in her hips and lower back during menstruation. Her menstrual flow is heavy and accompanied by very painful cramps.
Physical examination points to an enlarged or 'bulky' uterus which is also very tender. The woman's skin is pale and blood tests confirm that she is indeed anaemic. Her gynaecologic endoscopy scan is shown below.
